CMS’s national Doctors and Clinicians file can list one clinician on several lines: rows distinguish clinician, enrollment, group and address.
The dictionary separates an NPPES clinician NPI, a PECOS individual PAC ID and the enrollment ID identifying the source enrollment.
Group PAC ID identifies the PECOS group the clinician works with. CMS says this and the legal group name are blank when the address is not linked to a group.
For ENT reporting, we treat that as a CMS-listed association and seek separate primary evidence for employment or ownership claims.
The file has a 12-month claims lookback. This October 16 reading uses the dictionary’s version-history label, 9/10/26, rather than a newly verified roster.
