Federal payment-dispute administrators met with CMS and other federal officials on October 1 to discuss problems in the independent dispute resolution process, according to a CMS announcement published October 2.

CMS said users have raised concerns about ineligible disputes, unclear explanations for determinations, and delayed or missing payment after determinations. The agency convened certified IDR entities to hear how they are handling those issues and what operational changes they would suggest.

The announcement also described a planned transition from the Federal IDR Portal to an IDR Gateway in late 2026. That timing is the agency’s stated plan; the release does not report that the new system is already operating.

CMS said the federal departments will consider the participants’ input as they explore regulatory and operational responses. The announcement reports a discussion of process concerns, without ENT-specific reimbursement figures or a new specialty payment rule.

WHY WE’RE FOLLOWING IT / EDITORIAL CONTEXT

For ENT practices that encounter federal payment disputes, the announcement identifies process concerns raised with CMS. It supplies no specialty reimbursement results.
